*Your CPV application and 2 recommendation letters must be submitted ASAP and approved by the Amputee Coalition 2 weeks prior to the course date as you will need to complete a background check.
*Preferred application process is to be completed online at the link or QR code below.
*You must be an amputee at least one year to become a CPV.
